EMPLOYEE OBLIGATIONS AFTER
RETURNING FROM FOREIGN
COUNTRIES OR HOLIDAY ABROAD
In connection with the latest Health Protection Measure issued by the Czech
Ministry of Health, ŠKODA AUTO and the KOVO Trade Union would like to
remind you that every employee returning from abroad (including from a holiday)
is obliged to meet the conditions laid down in the aforementioned measure.
WHERE TO SEND
Vaccination completion certificates can be
generated at ocko.uzis.cz (after receiving
both doses for the two-dose vaccine / one
dose for the single-dose vaccine). If the
employee was vaccinated at a healthcare
centre outside of ŠKODA AUTO, they will
be required to provide their certificate in
electronic form, that means sending an
email with the scan of their COVID-19
vaccination certificate to this email:
Complete this Arrival Form
before entering the Czech
territory. The only exemption
applies to cross-border
commuters (AKA „pendlers“).

Employees are obliged to inform
the Employer of any stay abroad by
completing the Sworn Statement
immediately upon arrival
at their workplace.

deny access to the
worksite to any
employees who do
not submit their test
result along with the
The exception to submit a test belongs
to persons 14 days after completed
vaccination and persons who had
contracted COVID-19 in the last 180
days. However, the sworn statement
must still be provided. The same
exemption applies to cross-border
Employees that do not fall under one (or more) of these exemptions will submit the
sworn statement along with a negative test result - the test shall be taken at a healthcare
centre (a self-test will not be accepted). The term “test” means an antigen test (not older
than 48 hrs) or PCR test (not older than 72 hrs).
Employees will complete the sworn statement after each work absence if they
were abroad for more than 12 hours (24 hours in CZ-neighbouring countries). After
the end of the mass vacation utilization, i.e. from and including 15/8/2021, the
sworn statement obligation will apply to all employees.
Employees who spent their holiday in the Czech Republic will use the sworn
statement to confirm that they did not stay anywhere abroad. Give the
statement to the superior, and only show but keep your negative test result.
